*$2,000 Sign On Bonus Offered! **Mosaic Health* has an immediate opening for a *Social Work Care Manger* to join our integrated care team covering multiple site locations in the Mohawk Valley Region (located in Utica and Ilion, NY).The Mosaic Health Social Work Care Manager aims to improve patient health outcomes, and access to care by assessing and supporting patients to address social determinants of health needs, duties include but are not limited to:- Attending multi-disciplinary care team meetings to identify patients in need of support withany of the following needs: social determinants of health, health literacy, patientengagement, chronic and/or high risk medical, behavioral health or dental needs.- Providing outreach to patients to assess their social determinant of health needs through any ofthe following means: patient support questionnaire, warm handoff, electronic request, orpatient request.- Maintaining knowledge and understanding of community resources and organizations.- Obtaining initial New York State of Health Assistor Certification and completes requiredmandated training to maintain certification. As a Certified Application Counselor, supportingMosaic Health patients and/or community members to apply for health insurance in theNew York State Marketplace.- Assisting patients to apply for financial assistance programs for which they may eligible,including Mosaic Health Sliding Fee Discount program, pharmacy assistance, payment plans,and hardship programs.- Supporting patients to obtain transportation to healthcare appointments through MedicalAnswering Services (MAS) and other contracted transportation vendors.*Minimum Qualification Standards:** Bachelors degree in social work, Psychology, or related field, or 10 years of equivalentexperience* Case management experience* Valid NYS Drivers License as travel is required between other office locations* Ability to work flexible hoursWe offer a competitive salary and generous paid time off (18 paid days off and an additional 8 holidays).
