Description:Notre Dame Health Care- ND at HomeBereavement CoordinatorFull-Time (32 Hours) The Bereavement coordinator is responsible for overall management of a Notre Dame At Home- Hospice, Palliative Care and Pedi Pals bereavement program. Responsibilities include the planning, implementation, maintenance and provision of services necessary to patients, identified caregivers, staff, and the community at large. Notre Dame At Home's bereavement services extend for the time period of the patient's admission to hospice services until at least thirteen months (24 months for pediatric deaths) after the death of the patient.
